Insurance and Fees

Insurance:

As a licensed clinical psychologist, the psychotherapy services I provide are covered under most major medical health plans. I am not an in-network provider on any insurance plans. As an out-of-network provider, I collect fees directly from you, and provide you with a statement with which you can file a claim for reimbursement. Should you wish to file an insurance claim, I am happy to provide assistance to help you with that process. Please feel free to call me with any questions about insurance and fee policies.

Fees:

All sessions are billed at $215.00. For your convenience, I accept payment by cash, check, or credit card.

Good Faith Estimate Notification:

This notification is provided to you as required by the “No Surprises Act,” a part of the Consolidated Appropriations Act of 2021. You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

  • Under the law, I am required to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • I am required to give you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask me for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, contact me at: 703-519-6770 or drmaytorena@gmail.com, or visit www.cms.gov/nosurprises.
